Policies & Forms
Everything a family signs, in one place — for both Tie Dye Samurai and Tie Dye Jedi.
STEM+C is the 501(c)(3) nonprofit behind both robotics teams, so both teams use the same paperwork. There are two documents to sign — accepted once per season during registration — plus a privacy notice explaining what we do with your information. Every one of them can be read here or downloaded first.
Student & Family Team Agreement
How we treat each other, what we expect around attendance and communication, shop and safety rules, and how dues and family participation work. Signed by the student and a parent or guardian.
Replaces the separate Student Code of Conduct, Student Contract, and Parent Contract used in earlier seasons.
Consent, Assumption of Risk, Release & Emergency Authorization
Description of program hazards, assumption of risk, release of claims, safety obligations, emergency medical authorization, and an optional media choice. Signed by a parent or guardian for any participant under 18.
Please read this one carefully — it describes real hazards and affects legal rights. The media choice is optional and is not a condition of participation.
Privacy Notice
What we collect about students and families, why, how long we keep it, and who can see it. Nothing to sign — this one is for you to read.
These are in addition to FIRST's own forms
Every student on an FRC or FTC team also completes FIRST's consent and release through the FIRST registration system, separately from anything on this page. Your coach will walk you through it — it is required before a student can compete.
How signing works
Families accept both documents during team registration — there is nothing to print, scan, or mail. Your coach sends the registration link when a student joins.
The PDFs above are the same documents you will sign, so you can read them, keep them, or go through them with your student in advance. We would rather you signed something you had already read.
If you need a copy of something you have already signed, or you would rather complete the paperwork on paper, email info@stemplusc.org and we will sort it out.
